Job Description

Job Description An employer is hiring Remote RN Case Managers for a healthcare startup redefining how care is delivered for patients with neurological and endocrine conditions. We're looking for motivated, curious, and patient-first RNs who want to build meaningful relationships, think critically, and grow alongside a mission-driven team. If you're a motivated, "hungry" RN looking for a high-impact role with real growth this could be a great opportunity for you. This is a fully remote role where you'll play a key part in helping patients navigate complex, chronic conditions while contributing to a rapidly scaling care model. What You'll Do
  • Build ongoing relationships with patients through monthly virtual check-ins to track progress and adjust care plans
  • Support patients with neurological (Alzheimer's, Parkinson's, MS) and endocrine conditions (diabetes, thyroid disorders, metabolic conditions)
  • Coordinate care logistics including referrals, appointment scheduling, and medication reminders
  • Identify and recommend personalized resources (nutrition, exercise, sleep, lifestyle) tailored to each patient's condition
  • Partner with caregivers and clinical teams to ensure a seamless, supportive care experience
  • Educate patients on symptoms, disease progression, and modifiable risk factors in a clear, approachable way This is a role where you can grow quickly. As the company scales, you'll have opportunities to:
  • Mentor and train new LVNs and RNs
  • Support clinical escalations and decision-making

  • Active RN license (Compact license required)
  • 2-4 years of clinical experience (neurology and/or endocrinology experience preferred)
  • Strong interest in chronic disease management across neuro and endocrine conditions
  • Excellent communication skills- you can simplify complex clinical concepts for patients
  • Highly organized, proactive, and take ownership of patient outcomes
  • Coachable, collaborative, and excited to grow in a startup environment
  • Chronic Care Management experience
