13 Weeks Position Overview We are seeking an experienced Registered Nurse Case Manager / Clinic Nurse for a 13-week contract assignment in Juneau, Alaska. This hybrid role combines case management responsibilities with direct patient care in a primary care clinic setting. The ideal candidate will have recent case management experience, strong care coordination skills, and an active Alaska RN license at the time of submission. Key Responsibilities Manage assigned patient caseloads and coordinate comprehensive care plans. Collaborate with providers and interdisciplinary teams to improve patient outcomes. Conduct patient assessments, education, and follow-up outreach. Coordinate referrals, specialty care, and transitions of care. Participate in multidisciplinary case reviews and care conferences. Monitor acute and chronic care needs through EHR reminders and patient outreach. Serve as an advocate for patients and families throughout the care continuum. Perform clinic nursing duties including assessments, medication administration, documentation, and patient education. Participate in triage and respond to urgent or emergency situations as needed. Maintain compliance with safety, infection control, and annual competency requirements. Requirements Required Qualifications Active, unrestricted Alaska RN License (required at time of submission) Minimum 1 year of Case Management experience within the last 2 years Current BLS Certification Graduate of an accredited School of Nursing Proof of current Flu Vaccination Ability to work independently in a fast-paced clinical environment Preferred Qualifications Bachelor's Degree in Nursing (BSN) National Case Management Certification Experience in Ambulatory Care, Primary Care, or Clinic Settings Experience with
