Job Description
TEAM NY coordinates the delivery of home care services to Asian American patients in New York City and Long Island through a network of three Licensed Home Care Service Agencies (LHCSAs): ABI Healthcare, ATD Home Health Agency, and Life Improvement Homecare. Role Summary The RN Educator is responsible for translating clinical data and quality outcomes into comprehensive training and education programs across the organization. This role bridges quality improvement and staff development by ensuring that all training—ranging from new hire orientation to ongoing in-services—is directly informed by clinical trends, regulatory requirements, and organizational goals. This position plays a central role in onboarding, educating, and evaluating clinical staff, while driving improvements in patient outcomes through targeted, data-informed training initiatives.
Core Responsibilities:
Quality Assurance & Clinical Outcomes Integration Analyze and monitor key clinical indicators, including falls, hospitalizations, re-hospitalizations, and other VBP/PAH-related metrics Identify trends, gaps in care, and high-risk populations to inform training priorities Integrate clinical outcomes data into all training topics, ensuring education is aligned with real-time performance and quality improvement goals Present findings and recommendations in CQI and QA committee meetings RN Onboarding & Field Training Lead orientation programs for newly hired RNs, ensuring strong clinical and documentation foundations Conduct field training and mentorship, supporting new RNs in applying best practices in real patient settings Ongoing RN Education Plan and deliver quarterly clinical training sessions for Visiting RNs, focusing on: Advanced assessment skills, disease management, prevention of Potentially Avoidable Hospitalizations (PAH), documentation and compliance based on chart review findings. Conduct annual RN in-service trainings to reinforce regulatory requirements, clinical standards, and best practices Perform annual in-home supervision visits to assess RN competency, documentation quality, and adherence to care standards Competency & Performance Evaluation Assess staff competency following training sessions to ensure knowledge transfer into practice Use audits, supervision findings, and outcome metrics to evaluate training effectiveness Continuously refine training programs based on performance data and quality outcomes Care Coordinator Training Develop and host monthly training sessions for Care Coordinators, focusing on: Identifying clinical red flags, strengthening communication with the clinical team, understanding the impact of coordination on patient outcomes and VBP metrics Incorporate current outcome data into training scenarios and case discussions Training Development & Collaboration Partner with the Training & Development department to: Develop and update training materials for Home Health Aide (HHA) orientation and in-service programs, ensure consistency between RN-led clinical priorities and aide-level training content Create structured, outcomes-driven curricula and educational materials across all staff levels Qualifications & Skills Minimum of 2+ years of experience in a Home Care setting (LHCSA, CHHA, or MLTC). Experience with Quality Assurance, auditing, or clinical education is highly desirable. Licensed and registered in NYS as a Registered Professional Nurse Graduate of an accredited school of nursing with a diploma, baccalaureate, or master's degree

Ability to look at spreadsheets of raw numbers & understand the clinical story behind them.
Presentation Skills:
Confidence in public speaking and the ability to break down complex clinical concepts for non-clinical staff (Coordinators).
Regulatory Knowledge:
Understanding of NYS DOH regulations and VBP concepts. Proficiency with computer software and a willingness to learn EMR/EHR systems. A self-motivated and reliable work ethic with the ability to work both independently and as part of a team.
Language:
Bilingual proficiency in English and Chinese (Cantonese and/or Mandarin) is highly required.
