Newport, Rhode Island Job Summary:
AdviniaCare Newport is seeking a highly skilled and experienced MDS Coordinator to join our team. As an the MDS Coordinator, you will be responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for care and services provided within the facility. You will also oversee the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.
Essential Duties and Responsibilities:
Assess and determine the health status and level of care of all new admissions Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant changes Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to the facility according to regulatory time frames Maintain an accurate schedule of all MDS assessments, including proper reference dates throughout the resident's stay Perform data entry functions to ensure accurate data entry and electronic submission of MDS assessments Coordinate interdisciplinary participation in completing the MDS for each resident according to regulatory time frames Schedule and conduct resident care conferences in compliance with state and federal regulations Assist disciplines in formulating and revising care plans Evaluate resident care plans for comprehensiveness and individuality Participate in physical restraint reduction and fall management programs Set up documentation flow sheets for daily documentation of restorative nursing measures Perform or supervise the performance of restorative nursing staff Serves as a liaison with all therapists: physical, occupational, and speech
Qualifications:
Registered Nurse with current unencumbered state license 3-5 years of equivalent clinical experience Current CPR certification Long Term Care Experience preferred Ability to read, write, speak, and understand the English language
Physical Demands:
