In this vital role, you will manage the comprehensive assessment and documentation process for residents, ensuring compliance with federal and state regulations, including Medicare and NCQA standards. Your expertise will support optimal patient care, accurate medical records, and effective utilization management. This position offers an exciting opportunity to make a meaningful impact on resident outcomes while working within a dynamic healthcare environment that values innovation and excellence. Responsibilities Conduct thorough Minimum Data Set (MDS) assessments to evaluate residents' clinical status, functional capabilities, and care needs. Ensure timely completion and accurate documentation of all MDS sections, including RAI (Resident Assessment Instrument) processes, in accordance with regulatory guidelines. Collaborate with interdisciplinary teams—nursing, therapy, social services—to gather comprehensive clinical information for each resident's assessment. Review medical records, clinical documentation, and coding (ICD-10, CPT) to ensure accuracy and completeness for reimbursement purposes such as DRG (Diagnosis-Related Group) assignments. Manage the utilization review process by analyzing patient care plans, discharge planning needs, and ensuring appropriate level of care in compliance with Medicare and managed care policies. Maintain proficiency in EMR/EHR systems such as Cerner, Epic, Athenahealth, or eClinicalWorks to document assessments and facilitate seamless data flow. Stay current on industry standards including NCQA accreditation requirements, HIPAA regulations, and CMS guidelines to uphold compliance and quality standards. Qualifications Proven experience with MDS coordination within nursing homes or skilled nursing facilities; experience with home care or inpatient settings is highly desirable. Strong knowledge of medical terminology, anatomy, physiology, and clinical documentation improvement practices. Familiarity with
ICD-10/ICD
coding systems, CPT coding procedures, and Medicare regulations related to acute care and inpatient services. Experience working with EMR/EHR systems such as Cerner or Athenahealth; proficiency in utilizing health information management tools is preferred. Background in nursing or case management with critical care experience—ICU or Level I Trauma Center experience is a plus—demonstrating a solid understanding of patient care processes. Excellent organizational skills with attention to detail for documentation review and compliance tracking. Ability to communicate effectively across multidisciplinary teams while maintaining confidentiality in accordance with HIPAA standards.
