MDS Role:
As an RN MDS Coordinator, you will be responsible for coordinating the care planning process, educating staff on MDS processes, and ensuring compliance with regulations. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor resident progress, and make adjustments as needed. Your exceptional communication and organizational skills will be essential in maintaining accurate and timely documentation, facilitating effective communication between care team members, and prioritizing tasks to meet the needs of our residents.
MDS Responsibilities:
Coordinate the care planning process, including conducting initial and annual assessments, developing and implementing care plans, and monitoring resident progress Educate staff on MDS processes, including documentation requirements and care plan development Ensure compliance with regulations, including MDS regulations and facility policies Develop and maintain accurate and timely documentation, including care plans, progress notes, and incident reports Facilitate effective communication between care team members, including physicians, nurses, therapists, and other healthcare professionals Prioritize tasks to meet the needs of our residents, ensuring timely and efficient delivery of care Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance resident care
