The ideal candidate will provide high-quality patient care in-home for initial and follow-up assessments for client care plans and ongoing case management in the Charleston, South Carolina area. This role requires a strong understanding of medical documentation, anatomy, and nursing practices to ensure the well-being of our patients. Duties Conduct thorough patient assessments and develop individualized care plans. Maintain accurate medical documentation in compliance with healthcare regulations. Collaborate with interdisciplinary teams to coordinate patient care effectively. Provide education and support to patients and their families regarding health management when needed. Monitor patient progress and adjust care plans as needed. Respond promptly to emergencies in emergency situation. Experience Valid Registered Nurse (RN) license in the state of practice. Familiarity with laboratory procedures and emergency response protocols is advantageous. Strong understanding of anatomy and medical terminology. Excellent communication skills to interact effectively with patients, families, and healthcare professionals.
