Lutheran Hospice is currently recruiting for PRN Nurse Practitioners. We are seeking someone who is a compassionate, dedicated, and caring professional to join our team!
Responsibilities:
Provides advanced nursing care and treatment to hospice patients in consultation with and under the direct supervision of the hospice Medical Director In collaboration with the IDT, patient and family, provides consultation, assessments and evaluation of the plan of care Manage patient needs and respond to caregiver concerns as needed, to ensure comfort and care Counsels the patient and family in meeting nursing and related needs Provides health care instructions to the patient as appropriate per assessment and plan Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver
Qualifications:
Graduate of an accredited Nurse Practitioner program from a recognized university and obtained a Masters Degree in Nursing Current licensure in SC, CPR certification and valid driver's license and proof of insurance Minimum of three years nursing experience required. Prefer two years' experience in hospice, palliative care, or geriatric setting Must have reliable transportation for necessary travel throughout the service area Lutheran Homes of South Carolina is an equal opportunity employer.
