Serve with compassion. Be the difference. Job Summary Provides direct patient and family education through the continuum of care. Acts as a liaison between nursing and providers, ensuring high quality patient care while pursuing quality performance metrics. Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference. Facilitates care to include service navigation, transmission of patient information, and care coordination. Assesses, plans, develops, implements, and evaluates formal and informal educational opportunities and materials for patients, families, care providers, and the community. Coordinates care and educates physicians and other staff to facilitate the effective use of clinical and financial resources, focusing on admission, concurrent/continued stay and discharge planning needs. Collaborates and communicates with a diverse medical, nursing, and ancillary staff regarding strategies for improving and maintaining appropriate and timely resources. Serves as a resource to physicians, social workers and other staff for quality and utilization issues as well as discharge planning needs. Develops collaborative working relationships with medical staff members to coordinate data collection, processing, and project implementation. Serves as a clinical resource and consultant to provide answers to questions pertinent to area of practice and determines appropriate resources for nursing units, ancillary departments, and the community/public. Performs other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Requirements Education - Bachelor's degree in Nursing Experience - Three (3) years of experience in Behavioral Health I n Lieu Of Any incumbent in this position prior to 1-1-25 is required to have a nursing diploma or Associate's degree in nursing rather than a BSN. Required Certifications, Registrations, Licenses Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working. A valid driver's license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.
