Home Care Assessments Location:
Conway and the surrounding areas, being able to commute throughout Florence County, and Horry County is highly preferred
Pay:
$50 per visit (+ gas stipend)

PRN (Flexible Scheduling) Registered Nurse (RN) - Home Care Assessments Overview We are seeking a compassionate, detail-oriented Registered Nurse (RN) to join our non-medical home care agency. This position plays a vital role in ensuring high-quality, client-centered care through comprehensive in-home assessments and care plan development. The RN will meet with new clients in their homes, develop individualized care plans, conduct required supervisory visits, and provide in-office caregiver training. This role is ideal for a nurse who enjoys working independently in the field while also collaborating with an administrative team in an office setting or a nurse who has daytime availability and a flexible schedule to accommodate client assessments and required supervisory visits. Strong communication skills, organization, and accurate documentation are essential. Key Responsibilities Conduct comprehensive in-home client assessments Develop and implement individualized, person-centered care plans Complete required supervisory visits, including: Initial assessments, 30-day follow-up visits, Ongoing visits every four (4) months, as required by state regulations Ensure all documentation is completed accurately and in compliance with State regulations and HIPAA standards Identify client needs, safety concerns, and necessary care adjustments Communicate assessment findings and care plan updates to caregivers and office staff Beinging able to conduct mandatory annual DHEC in-service training for caregivers, and conduct in-service classes for new hires when needed Manage professional email communication related to client care and training Collaborate and coordinate communication across departments to ensure seamless service delivery Travel to client homes within the designated service area Qualifications Active and unrestricted Registered Nurse (RN) license in the state of South Carolina Minimum of 1 year of nursing experience preferred (home care experience a plus) Strong assessment, documentation, and organizational skills Excellent verbal and written communication abilities Comfortable working independently in the field Proficient with email and basic computer documentation systems Reliable transportation and willingness to travel throughout the Charleston area What We're Looking For Compassionate and client-focused mindset High level of professionalism and integrity Ability to maintain confidentiality and comply with HIPAA regulations Strong time management skills Team-oriented attitude while working independently If you are a motivated RN who values flexibility, autonomy, and the opportunity to make a meaningful impact in clients' homes, we encourage you to apply.
