Join the Peachtree Centre Team! Peachtree Centre is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our team. The ideal candidate is passionate about resident care, knowledgeable in MDS assessments, reimbursement processes, and regulatory compliance , and thrives in a collaborative skilled nursing environment. As an RN MDS Coordinator , you will play a key role in ensuring accurate resident assessments, care planning, quality outcomes, and reimbursement optimization while supporting our commitment to exceptional resident-centered care.

Responsibilities:

✔ Complete and coordinate MDS assessments in compliance with CMS and state regulations ✔ Manage assessment schedules and ensure timely submission of required documentation ✔ Develop, update, and coordinate resident care plans with the interdisciplinary team ✔ Review clinical documentation to support reimbursement accuracy and compliance ✔ Monitor quality measures and support performance improvement initiatives ✔ Collaborate with nursing, therapy, social services, dietary, and physician teams ✔ Participate in survey preparation and maintain regulatory compliance standards ✔ Provide education and support to staff regarding documentation and MDS processes ✔ Analyze trends related to reimbursement, quality measures, and resident outcomes

Qualifications:

Required:

Current, active RN license in South Carolina Minimum 2 years nursing experience , preferably in long-term care or skilled nursing Knowledge of

MDS 3.0, CMS

regulations, Medicare, Medicaid, and care planning Strong organizational, communication, and time management skills Ability to manage multiple deadlines and maintain accuracy

Preferred:
