Job Description Benefits:
South Carolina State Health Insurance South Carolina State Retirement Competitive pay Paid Parental Leave The Case Manager's primary function is to help patients navigate their healthcare journey by coordinating their care, advocating for them, and ensuring they receive the best care possible. Responsible for functions such as Utilization Review, Discharge Planning, Social Services, and Resource Management. Duties include but are not limited to: Review of patient's medical record, health history, and needs to create a care plan that helps them achieve their goals. Develop, document, and implement plans, which provide appropriate resources to address the social, physical, mental, emotional, spiritual, and supportive needs of the patient. Review criteria to assess and document the appropriateness of admission, continued stay, and level of care Assists patients, their families, and physicians with post-acute placement Advocate for patients ensuring they are receiving the best care in a cost-effective way. Refers cases that do not meet admission and/or continued stay criteria to the designated Physician Educate patients and families about treatment options and health status Performs additional assignments and tasks as assigned by Case Management Department Head.
Experience:
Minimum of three years of medical practice/clinical management experience. Other experience may be substituted in place of practice/clinical management
Additional Job Description Education:
Graduated from an accredited school of nursing. Current licensure by the South Carolina State Board of Nursing. Bachelor's degree (BSN) preferred.
Licensure, Certifications, Registrations:
South Carolina Registered Nurse licensure, Basic Life Support (BLS) with American Heart Association (AHA) required.
