Responsibilities:
Conduct initial client assessments and ongoing supervisory visits Develop and update individualized care plans Monitor client conditions and communicate changes to healthcare providers Train, support, and evaluate caregivers Review documentation for accuracy and compliance Collaborate with clients, families, physicians, and community healthcare partners Ensure compliance with agency policies, HIPAA, and state regulations
Qualifications:
Active RN Minimum 2 years of nursing experience (home care experience preferred) Strong assessment, communication, and organizational skills Ability to work independently and manage multiple priorities Reliable transportation for local client visits Why Join AllCare?
