Job Description
Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. Hospice Registered Nurse (RN) - Walterboro, SC A rewarding opportunity is available for a skilled Hospice Registered Nurse to deliver compassionate, patient-focused end-of-life care in home and residential settings. This contract role involves managing hospice care plans with sensitivity and clinical expertise, supporting patients and their families through challenging times with dignity and respect.
Key Responsibilities:
Conduct thorough nursing assessments and develop personalized hospice care plans. Provide skilled nursing focused on symptom relief, pain management, and patient comfort. Administer medications and treatments according to physician orders and hospice protocols. Monitor patient conditions, reporting changes promptly to the medical director and care team. Educate patients and families on disease progression, comfort care, and medication use. Collaborate closely with social workers, chaplains, CNAs, and volunteers to meet physical, emotional, and spiritual needs. Complete accurate, timely electronic documentation using Home Care Home Base (HCHB). Participate in interdisciplinary case conferences and quality improvement initiatives. Adhere to all regulatory standards and hospice policies.Qualifications:
Active RN license in good standing for the state of South Carolina. Minimum one year recent hospice nursing experience. Proficiency in Home Care Home Base (HCHB). Strong clinical assessment and communication skills. Understanding of pain management and family support in end-of-life care. Reliable transportation with valid driver's license and insurance. Ability to manage a field-based schedule independently. Compassionate with emotional resilience.Preferred:
Bachelor of Science in Nursing (BSN). Experience in palliative or hospice care. Knowledge of Medicare hospice regulations and documentation requirements.Schedule:
Full-time, Monday-Friday, 8:00 AM-5:00 PM, with occasional weekend/on-call duties. Approximately 4-5 patient visits daily in homes, assisted living, and nursing facilities. Daily travel within assigned service area; mileage reimbursement provided.Benefits:
