Complete accurate, timely electronic documentation using Home Care Home Base (HCHB). Participate in interdisciplinary case conferences and quality improvement initiatives. Adhere to all regulatory standards and hospice policies.

Qualifications:

Active RN license in good standing for the state of South Carolina. Minimum one year recent hospice nursing experience. Proficiency in Home Care Home Base (HCHB). Strong clinical assessment and communication skills. Understanding of pain management and family support in end-of-life care. Reliable transportation with valid driver's license and insurance. Ability to manage a field-based schedule independently. Compassionate with emotional resilience.

Schedule:
