On-call coverage every weekend from Friday at 5:00 PM through Monday at 8:00 AM Position Summary:
BST Family Hospice is seeking a compassionate and clinically skilled PRN Weekend On-Call Hospice Registered Nurse (RN) to provide high-quality, patient-centered care to hospice patients residing within our partnered Brookstone Terrace Assisted Living communities . This role is responsible for managing all after-hours and weekend clinical needs , ensuring continuity of care, timely symptom management, and supportive communication with families and facility staff. The ideal candidate demonstrates strong clinical judgment, responsiveness, and a commitment to delivering end-of-life care with dignity and compassion. Key Responsibilities:
Provide on-call hospice nursing coverage for all assigned patients during scheduled weekend hours Respond promptly to after-hours calls , triage patient needs, and determine appropriate interventions Perform in-person visits as needed for symptom management, crises, or patient/family support Complete comprehensive nursing assessments and document all care in the EMR (e.g., MatrixCare) Manage and implement physician orders , including medication adjustments and treatments Provide end-of-life care , including support during active dying and post-mortem care Collaborate with ALF staff, hospice interdisciplinary team, and Medical Director to ensure coordinated care Educate families and caregivers on disease progression, symptom management, and comfort measures Ensure compliance with CMS Conditions of Participation, CHAP standards, and DHEC regulations Participate in on-call handoff communication to ensure seamless continuity of care Clinical Duties May Include:
Pain and symptom management (e.g., dyspnea, agitation, secretions) Wound care and dressing changes Medication administration and education Management of terminal symptoms and crisis intervention Pronouncement of death (per agency policy and state regulations) Coordination with funeral homes and post-mortem processes Qualifications:
Current Registered Nurse (RN) license in South Carolina Minimum 1-2 years of clinical nursing experience (hospice or home health preferred) Strong assessment and critical thinking skills Ability to work independently and make sound clinical decisions Excellent communication and interpersonal skills Valid driver's license and reliable transportation Preferred Qualifications:
Previous hospice or palliative care experience Experience working within Assisted Living Facilities (ALFs) Familiarity with end-of-life symptom management protocols Core Competencies:
