Job Description

Job Overview We are seeking a dedicated and compassionate Registered Nurse (LTC) to join our long-term care (LTC) team. In this vital role, you will provide high-quality nursing care to residents in a supportive, patient-centered environment. Your expertise will ensure residents receive personalized care that promotes their health, safety, and well-being. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical and emotional support while working within a dynamic healthcare setting. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history reviews to develop individualized care plans. Administer medications and injections accurately, including IV infusions and other advanced therapies such as botulinum toxin treatments when applicable. Monitor residents' conditions closely, managing acute pain, infectious diseases, or chronic illnesses with evidence-based interventions. Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and compliance with regulatory standards like NCQA and HIPAA. Perform specialized procedures such as suturing, catheterization, ostomy care, and tube feeding while maintaining aseptic techniques. Document all patient interactions thoroughly using electronic health record (EHR) systems like Cerner or Athenahealth to ensure accurate medical records. Provide education and support to residents and their families on health coaching, medication administration, and disease management strategies. Requirements Valid RN license with current registration in the state of practice. Proven experience in long-term care (LTC), nursing homes, or assisted living facilities. Critical care experience including ICU or emergency room exposure is highly desirable. Familiarity with EMR/EHR systems such as Cerner or Athenahealth for documentation purposes. Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), laboratory procedures, and diagnostic evaluation techniques. Ability to perform complex procedures like venipuncture, IV insertion, sterile processing, and respiratory support including ventilator management. Strong understanding of infection control protocols and aseptic techniques essential for safe patient care. Excellent assessment skills with the ability to triage effectively in urgent situations involving trauma medicine or acute pain management. Experience working with diverse populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia care needs. Certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and relevant specialty training are preferred but not mandatory. Pay:
$34.00 - $48.00 per hour
