RESPONSIBILITIES

This position serves in a dual role, with approximately 50% of time dedicated to case management duties and 50% focused on client medical coordination and support. As part of the LSSSD team, the case manager supports and strengthens individuals and families by developing client-centered case plans, providing ongoing assessment, and assisting clients in accessing community resources to meet their needs. Responsibilities include maintaining accurate case documentation; building collaborative relationships with community partners and service providers; coordinating medical, dental, and vision appointments; ensuring annual exams are completed; assisting with Medicaid eligibility; supporting medication management processes in collaboration with nursing staff and pharmacy providers; obtaining medical records and treatment consents; reviewing Medication Administration Records (MARs) for accuracy; and transporting clients to appointments as needed. This position participates in the on-call rotation system.

QUALIFICATIONS

A bachelor's degree in social work or a related field, is required. Bi-lingual is a preferred but not required. Must be med-aid trained, familiar with electronic Medication Administration Record (MAR). Must be at least 21 years of age, is required. Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes. Requires a valid auto-insurance, a valid driver's license or the ability to obtain one with approved driver status. Only applications submitted through our company page will be considered.
