Key Responsibilities:
Provide skilled nursing care to patients in their homes as per physician orders and care plans Perform comprehensive patient assessments and develop individualized care plans Coordinate care with physicians, therapists, and interdisciplinary healthcare teams Educate patients and families on disease process, medications, and home care management Maintain accurate documentation using EPIC charting system Monitor patient condition and report any changes promptly Participate in call coverage every 3rd-4th weekend and 1 weekday as assigned Ensure compliance with home health policies, safety standards, and infection control practices Support continuity of care and discharge planning
Requirements:
Minimum 1 year of RN experience required Home Health Case Management experience preferred
Certifications Required:
BLS Active RN License #healthcare
Pay:
$40.00
