Job Title:
Travel RN Long Term Care. Job Overview We are seeking a dedicated and highly skilled Long-Term Care Registered Nurse to provide comprehensive nursing care within our long-term care facility. The ideal candidate will possess a strong background in geriatrics, memory care, and complex patient assessments, with a commitment to delivering compassionate, patient-centered care. This role involves coordinating with multidisciplinary teams to ensure optimal health outcomes for residents, managing medical documentation accurately, and adhering to all regulatory standards. The successful applicant will demonstrate proficiency in EMR systems, medication administration, and specialized nursing procedures pertinent to long-term and assisted living environments.

12-hours
Days Shift Details:
6:00 AM - 6:30 PM 48 hours/week. (4
  • 12) 8-week assignment Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations tailored to elderly and developmentally disabled populations.
Administer medications via various routes such as injections, IV infusions, and tube feeding while ensuring compliance with HIPAA regulations and safety protocols. Develop individualized care plans in collaboration with physicians, case managers, and family members, incorporating elements of pain management, dementia care, and behavioral health support. Monitor residents' health status continuously, respond promptly to acute pain or emergency situations, and coordinate triage procedures effectively. Maintain accurate medical records using EMR/EHR systems such as Cerner or Athenahealth; document all patient interactions and treatments thoroughly. Provide education on health coaching topics including medication adherence, nutrition, and disease management for residents and their families. Perform specialized procedures such as suturing, ostomy care, catheterization, phlebotomy, and sterile processing when necessary. Collaborate with interdisciplinary teams to ensure compliance with NCQA standards and facilitate discharge planning or transition to post-acute or hospice care when appropriate. Requirements Valid RN licensure in the state of practice with current CPR/BLS certification. Extensive experience in long-term care settings including nursing homes, assisted living facilities, or memory care units. Proficiency in EMR/EHR systems such as Cerner or Athenahealth; familiarity with
ICD-10/ICD-9
coding for accurate documentation. Demonstrated expertise in managing complex cases involving acute pain management, infectious disease care, trauma medicine (Level I & II trauma center experience preferred), and behavioral health issues. Critical care experience including ICU or emergency room background is highly desirable; knowledge of ventilator management is advantageous. Strong understanding of infection control practices and aseptic techniques essential for safe patient care. Ability to work effectively with individuals with developmental disabilities or dementia; experience in geriatrics or hospice & palliative medicine is beneficial. Excellent communication skills combined with the ability to perform physical examinations and assessments accurately. Knowledge of Workers' Compensation law relevant to workplace injuries; familiarity with medical imaging or laboratory experience is a plus.
