Job Description

Home in the Heart Healthcare Agency Hardin County, Tennessee Home in the Heart Healthcare Agency is seeking a compassionate and experienced Registered Nurse (RN) to perform supervisory assessments and oversight visits for client receiving Homemaker/Home Health Aide services. This is a PRN/Part-Time position and is not guaranteed full-time hours. Assessments are completed on an as-needed basis throughout Hardin County and surrounding areas. Position Summary The RN Supervisory Assessment Nurse conducts initial assessments, supervisory visits, reassessments, and care plan reviews to ensure veterans receive safe, appropriate, and quality home care services. The nurse serves as a liaison between the client, family members, caregivers, and agency staff while ensuring compliance with VA and state regulations. Essential Duties and Responsibilities Conduct initial in-home assessments for newly referred veterans. Complete supervisory nursing visits for veterans receiving Homemaker/Home Health Aide services. Perform reassessments to determine continued eligibility and ongoing service needs. Evaluate the veteran's physical condition, functional abilities, safety risks, and home environment. Review medical records, physician orders, and previous assessments to identify changes in condition and care needs. Develop, update, and maintain individualized care plans. Provide clinical oversight and guidance to caregivers and home health aides. Assess caregiver performance and recommend additional training when needed. Educate veterans and family members regarding available services, safety measures, disease management, and preventive care strategies. Document assessments, supervisory visits, and care plan updates accurately and timely. Communicate significant findings and recommendations to physicians, case managers, VA representatives, and agency leadership. Monitor for changes in condition that may require additional services or physician intervention. Ensure compliance with agency policies, VA requirements, and Tennessee nursing regulations. Participate in quality assurance and performance improvement activities. Perform other duties as assigned. Minimum Qualifications Current, unrestricted Tennessee Registered Nurse (RN) License. Valid driver's license and reliable transportation. Ability to travel throughout Hardin County and surrounding service areas. Strong clinical assessment and documentation skills. Excellent communication and interpersonal skills. Ability to work independently with minimal supervision. Preferred Qualifications Experience in one or more of the following: Home Care/Home Health Geriatric Nursing Veterans Care Case Management Discharge Planning Medical-Surgical Nursing Long-Term Care Community-Based Nursing Knowledge, Skills, and Abilities Strong critical thinking and clinical judgment. Ability to assess and evaluate patient care needs. Excellent time management and organizational skills. Ability to manage multiple assessments and documentation requirements. Proficiency in Microsoft Word, Excel, Outlook, and electronic health records. Ability to build positive relationships with veterans, families, caregivers, and interdisciplinary team members.
