Coverage Areas:
  • Clearfield County, PA
Centre County, PA Primary Responsibilities:
  • Provide skilled nursing care in the home health setting under physician orders and in compliance with applicable laws, regulations, and agency policies
  • Perform OASIS assessments, start of care visits, return visits, and ongoing patient evaluations
  • Develop, implement, and revise individualized care plans based on patient needs and clinical findings
  • Deliver high-quality clinical care including IV therapy, wound care, and wound VAC management
  • Conduct venipuncture and administer treatments as required
  • Provide patient and caregiver education on disease management, medications, and home care techniques
  • Ensure accurate, timely, and compliant documentation in accordance with agency and payer requirements
  • Coordinate care with physicians and interdisciplinary healthcare team members to ensure continuity and quality outcomes
  • Monitor patient conditions and identify changes requiring intervention
  • Travel daily to patient homes across assigned coverage areas and provide care in multiple locations within each shift
  • Participate in on-call and weekend rotation as required
  • Ensure compliance with all home health policies, procedures, and safety standards
Qualifications:
  • Active Registered Nurse (RN) license in the state of Pennsylvania required
  • Current BLS certification preferred
  • Minimum home health experience required
  • Experience with OASIS documentation required
  • Hands-on experience with IV therapy, wound care, wound VACs, venipuncture, and patient disease education required
  • Strong clinical assessment, documentation, and care coordination skills required Why work for Adecco?

Pay Details:
