We are seeking a compassionate and skilled Registered Nurse (RN) to join our team at Windcrest Health & Rehabilitation, a 120-bed premier skilled nursing, rehabilitation care center and certified Alzheimer facility in Abilene, Texas. As a RN with our team, you will play a vital role in providing high-quality, patient-centered care to our residents. About the
Role:
As a Registered Nurse (RN) at Windcrest Health & Rehabilitation, you will be responsible for: Providing direct patient care to our residents, including assessing, planning, implementing, and evaluating individualized care plans Developing and maintaining effective relationships with patients, families, and healthcare team members Collaborating with interdisciplinary teams to ensure seamless transitions and optimal care outcomes Participating in quality improvement initiatives to enhance patient satisfaction and clinical outcomes Maintaining accurate and thorough records of patient care and treatment
Responsibilities:
Conduct initial and ongoing assessments of patients, identifying and addressing physical, emotional, and psychological needs Develop and implement individualized care plans, incorporating patient goals, preferences, and values Provide skilled nursing interventions, including wound care, medication management, and Vital Sign monitoring Collaborate with physical, occupational, and speech therapists to ensure comprehensive rehabilitation and post-acute care Educate patients, families, and caregivers on disease processes, treatment plans, and self-care techniques Participate in routine care conferences and interdisciplinary team meetings to share information and coordinate care
