Job Description

Hospice Registered Nurse (RN) Case Manager - Alice, Texas Join a compassionate team that is dedicated to providing exceptional end-of-life care to patients and families in the Alice, Texas area. We are seeking an experienced and caring Hospice Registered Nurse (RN) Case Manager who is passionate about making a meaningful difference in the lives of others. Position Summary The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients while working closely with an interdisciplinary team to ensure comfort, dignity, and quality of life. This role requires strong clinical skills, compassionate communication, and the ability to work independently in the field. Responsibilities Complete comprehensive nursing assessments and develop individualized care plans Coordinate patient care with physicians, social workers, chaplains, aides, and other team members Provide skilled nursing care and symptom management Educate patients and families regarding disease progression, medications, and plan of care Perform admissions, routine visits, on-call support, and discharge planning as needed Maintain accurate and timely documentation in compliance with hospice regulations Participate in interdisciplinary team meetings and care coordination Provide emotional support and compassionate guidance to patients and families Qualifications Current Texas Registered Nurse (RN) license in good standing Minimum one (1) year RN experience required Hospice or home health experience preferred Strong assessment, communication, and organizational skills Ability to work independently and manage a patient caseload effectively Valid driver's license and reliable transportation Benefits Competitive salary Mileage reimbursement Health, dental, and vision insurance Paid time off 401(k) with company match Supportive team environment Opportunities for professional growth and advancement Why Join Us?
