The Hospice RN Case Manager is a licensed Registered Nurse responsible for overseeing the provision of high-quality hospice care to patients and families. This role ensures comprehensive, compassionate, and coordinated care while supervising nursing staff, maintaining compliance with regulatory standards. The Hospice RN Case Manager provides direct patient case management and supports the delivery of hospice services consistent with the mission, vision, and values of the organization.
Qualifications:
Current, unrestricted Registered Nurse (RN) license in the state of practice.

Minimum of 23 years of clinical nursing experience; hospice or palliative care experience strongly preferred.

Prior supervisory or leadership experience preferred.

Knowledge of hospice philosophy, end-of-life care, and applicable regulations.

Strong clinical assessment, critical thinking, and problem-solving skills.

Excellent interpersonal, communication, and leadership abilities.

Valid drivers license and reliable transportation (for home visits, as required).
