- A rewarding opportunity is available for an experienced Registered Nurse (RN) Case Manager to join a growing home health team serving patients throughout the South Austin community.
This role combines hands-on patient care with case management responsibilities, allowing you to guide patients through their healthcare journey while helping them remain safe and independent at home. As an RN Case Manager, you'll coordinate and oversee individualized care plans, collaborate with interdisciplinary healthcare professionals, and build meaningful relationships with patients and their families. - Key Responsibilities
- + Conduct comprehensive patient assessments within the home setting.
+ Develop, implement, and oversee individualized plans of care. + Coordinate services and communicate effectively with physicians and interdisciplinary team members to ensure continuity of care. + Monitor patient progress and modify care plans as clinical needs evolve. + Educate patients and caregivers regarding medications, diagnoses, treatment plans, and health management strategies. + Maintain timely, accurate, and compliant clinical documentation. + Advocate for patient needs while promoting safety, independence, and quality outcomes. + Manage an assigned caseload while prioritizing compassionate, patient-centered care.
- Qualifications
- Required Experience
- + Previous experience working as a Registered Nurse is required. + Home health experience is strongly preferred. + OASIS experience is required. + Reliable transportation for home visits. + Ability to work independently while effectively managing multiple patient priorities. + Excellent assessment, communication, organizational, and critical-thinking skills.
- Education, Licensure & Certifications
- + Graduate of an accredited school of professional nursing. + Active and unrestricted
- Registered Nurse (RN)
- license in the State of Texas. + Current CPR certification required and maintained throughout employment. + Valid driver's license and access to a reliable, insured vehicle suitable for travel throughout the assigned territory. + Ability to travel to patient homes and designated care locations as needed.
- Compensation & Schedule
- + Annual compensation ranges from $83,000 to $102,000, based on experience and qualifications.
Point Per Basis. + Full-time position with a consistent schedule of Friday through Sunday from 8:00 AM to 5:00 PM. + Ideal for nurses seeking a weekend-focused schedule while maintaining regular daytime hours. + No on-call responsibilities, allowing for greater work-life balance and predictability outside of scheduled shifts.
