We are seeking a compassionate and dedicated Registered Nurse to join our hospice care team, where you will serve as a vital guide for patients and families navigating their most profound journey. Where You Will Serve We are expanding our reach throughout South Austin, Dripping Springs, Kyle, Buda, and the surrounding communities. The Role As a Hospice RN Care Manager, you will be building trusting relationships in the homes of those who need us most. You will provide comprehensive, compassionate care, ensuring every patient's final days are filled with comfort and quality of life. You will play a vital role in assessing, planning, and implementing individualized care plans for multiple patients in varied locations, while providing the support and education that families desperately need. Conduct initial and ongoing comprehensive assessments of the terminal disease's impact on patients' physical, functional, psychosocial, and environmental needs. You will listen, observe, and document findings to develop care plans that truly reflect the patient's story.Care Planning:
Implement and document personalized care plans to ensure quality and continuity. You will make necessary revisions based on patient needs and outcomes, ensuring care evolves as the journey progresses.Education:
Be a trusted consultant and educator for patients and families. Explain complex disease processes, teach self-care techniques, guide end-of-life care, and address dietary needs. You will also provide training for other staff as needed.Care Coordination:
Determine the scope and frequency of services needed based on the patient's condition. You will coordinate seamlessly with our interdisciplinary team to prioritize care needs and ensure everyone is on the same page.Preventive Care:
Initiate appropriate preventive and rehabilitative nursing procedures, applying strict infection control practices to prevent contamination and transmission of disease.Clinical Documentation:
Prepare clinical and progress notes that reflect progress toward established goals. You will ensure compliance with third-party payor requirements while maintaining the highest standards of patient care documentation.Communication:
Maintain effective, empathetic communication with patients, families, team members, and other healthcare professionals. You will keep physicians informed of changes in patient needs and outcomes.On-Call Services:
Provide on-call services during business hours to ensure patient and family support during emergencies, knowing you are never alone in your responsibilities.Professional Growth:
Participate in hospice-sponsored in-service programs, maintain your professional growth, and contribute to the hospice's Quality Assessment and Performance Improvement (QAPI) program.What You Will Bring, Do, and Own Compassion:

You have a passion for hospice or palliative care, and you believe that the end of life deserves the same level of care and attention as the beginning.What You Will Need To Have Current RN License:
Active in Texas and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.Experience:
2 years of nursing experience preferred (1 year required). Experience in hospice or palliative care is highly valued.Transportation:
A valid driver's license, insurance, and reliable transportation to and around the South Austin area (including South Austin, Dripping Springs, Kyle, Buda, and surrounding areas).Mindset:
Maturity and the ability to deal effectively with the emotional and physical demands of the job.Skills:
