• Provide skilled nursing visits in accordance with physician orders and individualized care plans
• Complete comprehensive patient assessments and ongoing evaluations
• Educate patients and caregivers regarding treatment plans, medications, and disease management

• Monitor patient progress and communicate changes in condition as appropriate
• Ensure quality care and adherence to home health standards and regulations Qualifications
• Graduate of an accredited school of professional nursing
• Current Registered Nurse (RN) license in the State of Texas
• Minimum of one year of RN experience
• Geriatric Home Health experience required
