Full-time What You Will Do Holistic Care Coordination:
Serves as the primary liaison across multidisciplinary teams to implement, monitor, and adapt comprehensive patient care plans in alignment with organizational protocols.
Clinical Excellence & Monitoring:
Executes complex physician and nursing orders with precision, maintaining rigorous oversight of patient conditions and ensuring timely, accurate documentation and communication.
Strategic Discharge Planning:
Leads the assessment and coordination of patient discharge requirements, collaborating with the healthcare team to ensure seamless transitions and continuity of care. What You Will Need Licensure / Certification /
Registration:
Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Pediatric Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
Education:
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. Additional Preferences Prefer Pediatric w/some Ambulatory experience. Respiratory experience is also strongly preferred.
