Job Summary We are seeking a dedicated and compassionate Registered Nurse to join our home health team. The Registered Nurse will be responsible for assessing patient needs, implementing care plans, using kinnser software and collaborating with other healthcare professionals to ensure optimal patient outcomes. Duties Conduct comprehensive assessments of patients and admission of patients in their homes. Know How to utilize EMR System Kinnser/Wellsky Work out of the office location Part-time Educate patients and their families on health management, discharge planning, and the use of Durable Medical Equipment (DME). Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients in their homes. Maintain a safe and sterile environment by following established infection control practices. Experience Candidates should possess the following skills and experience: Previous experience in home health Proficiency in infection control measures and aseptic techniques. Experience working with diverse patient populations in outpatient settings. Strong communication skills to effectively interact with patients, families, and healthcare team members. A valid nursing license is required.
