Job Description

Job Summary We are seeking an experienced MDS Nurse to join our skilled nursing team. The MDS Nurse is responsible for completing accurate and timely MDS assessments, coordinating resident care plans, and ensuring compliance with Medicare, Medicaid, and state and federal regulations. Responsibilities Complete and submit MDS assessments accurately and on time. Coordinate interdisciplinary care plan meetings. Ensure compliance with CMS, Medicare, Medicaid, and Texas regulations. Review resident charts and documentation for accuracy. Work closely with nursing, therapy, and physician teams. Monitor reimbursement and case mix to maximize appropriate reimbursement. Stay current on MDS guidelines and regulatory changes. Qualifications Current Texas RN or LVN license in good standing. Previous MDS experience in a skilled nursing facility required. Knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines. Strong organizational, communication, and computer skills. Ability to work independently while meeting deadlines.
