Full Job Description Job Purpose:
The Wound Care Nurse is responsible and accountable for the assessment, planning, implementation, and evaluation of wound care. This role involves providing direct patient care and education to assigned patients to ensure optimal healing and recovery.
Key Responsibilities:
Assess the needs of patients with basic and complex wounds to determine appropriate care strategies. Develop a comprehensive wound management plan tailored to individual patient needs and assist staff in its implementation. Document the wound care plan and any revisions made to ensure continuity of care and accurate patient records. Coordinate the evaluation of wound care, monitoring, and documenting patients' responses to treatment. Obtain and document pictures of wounds upon admission and weekly to track progress and inform treatment adjustments.
Required Education:
Bachelor's degree or a combination of an associate's degree in an approved Registered Nurse program with meaningful and appropriate experience.
Required Experience:
Experience in assessing, planning, implementing, and evaluating wound care. Experience in providing direct patient care and education to assigned patients.
Preferred Experience:
Two (2) years of experience in an acute or LTACH (Long-Term Acute Care Hospital) setting is preferred.
Required Skills and Abilities:
Current Registered Nurse license in the state where practicing is required. Current Basic Life Support (BLS) certification. Advanced Cardiovascular Life Support (ACLS) certification within 6 months of hire. Proficiency in electronic medical records and documentation.
Preferred Skills and Abilities:
Certified Wound Ostomy Nurse (CWON) certification.
