Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.Qualifications:
Licensed Vocational Nurse, per the state requirements. 1 year of Home Health experience Good communication skills, assessment skills, and strong clinical documentation skills. Ability to read, write, and speak the English language. Reliable transportation. Valid and current auto liability insurance.Essential functions:
Under the direction of the RN, assist in identifying the patient's physical, psychosocial, and environmental needs. Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. Provide care utilizing infection control measures that protect both staff and patient (OSHA). Records treatments administered in medical record. Records clinical documentation according to accepted regulatory, corporate and professional guidelines. Participates in weekly case conferences. Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress. Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment. Participates in in-service training programs. Responsible for assuring patient safety. Complies with state regulations and national standards All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
