Job Description

Position Summary:
OpusCare is seeking an experienced and compassionate Hospice Representative to build and maintain strong referral relationships within the healthcare community. This individual will serve as the bridge between referral sources, patients, families, and our clinical team to ensure smooth transitions into hospice care. The ideal candidate must have prior healthcare, hospice, home health, or patient relations experience and possess exceptional communication and relationship-building skills.
Responsibilities:
Develop and maintain relationships with hospitals, physicians, case managers, skilled nursing facilities, ALFs, and community partners Educate referral sources on hospice philosophy, eligibility, and services Meet with patients and families to explain hospice services with compassion and professionalism Coordinate referrals and admissions with the clinical and intake teams Conduct in-services, presentations, and community outreach events Maintain territory management and referral activity documentation Ensure compliance with HIPAA and company policies Represent OpusCare professionally within the healthcare community
Qualifications:
Minimum of 2 years of experience in hospice, healthcare sales, home health, or patient relations Strong understanding of hospice philosophy and end-of-life care Excellent interpersonal and communication skills Ability to build trusted relationships with referral partners Self-motivated, organized, and goal-oriented Reliable transportation and willingness to travel throughout assigned territory Bilingual English/Spanish preferred
