Join us as a PRN RN—where Clinical excellence meets purpose! Acara Health, one of the largest providers of quality in-home health care in the state of Texas, is seeking a PRN Registered Nurse. Acara Health's Personal Care provides industry-leading in-home care in the markets of Central and South Texas.
Job Overview:
The PRN Registered Nurse (RN) role plays a critical part in supporting regulatory compliance and person-centered care within the Texas Medicaid waiver environment, with a primary focus on the CLASS (Community Living Assistance and Support Services) program. The position is responsible for conducting annual nursing assessments and health status reviews to evaluate individuals' medical needs, functional abilities, health risks, and ongoing service requirements. This role also contributes to person-centered care planning by identifying changes in health status, making clinical recommendations, and collaborating with interdisciplinary teams. Required Qualifications Active Texas Registered Nurse (RN) license in good standing. Experience with Medicaid waiver programs, preferably the Texas CLASS Waiver program. Knowledge of person-centered planning and interdisciplinary service delivery. Experience completing nursing assessments, health status reviews, and care planning activities. Strong documentation skills and attention to detail. Ability to work independently across multiple service areas. Preferred Experience Prior CLASS, HCS, TxHmL, home health, case management, or community-based services experience. Familiarity with HHSC and TMHP documentation requirements. Experience working with individuals with intellectual and developmental disabilities Key Attributes Critical thinking and clinical assessment skills. Attention to Detail Strong organizational and time management abilities. Ability to identify changes in health status and communicate recommendations effectively. Professional communication skills with individuals, families, providers, and interdisciplinary teams. Flexible schedule to support assessments across multiple geographic markets. Willingness and ability to travel throughout Del Rio, Eagle Pass, Laredo, and Austin. Ability to work with minimal supervision while maintaining regulatory compliance.
