RN Case Manager LoveWell Hospice Denton, TX Job Details $87,000 - $93,000 a year 22 hours ago Qualifications Hospice experience Patient advocacy Full Job Description Job Summary We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including patient homes and assisted living facilities. This role requires a strong commitment to patient advocacy, effective communication skills, and the ability to work collaboratively with multidisciplinary teams.
PREVIOUS HOSPICE EXPERIENCE PREFERRED.
BetterRx experience preferred. FireNote EMR and experience preferred. Our offices are located in Allen, TX but the areas of the Metroplex in need of RN coverage are as follows: Lewisville, Denton, Carrollton, Flower Mound. Responsibilities Assess and monitor patient health status, including vital signs and medical history. Administer medications and treatments as prescribed by physicians. Develop and implement individualized care plans for patients, particularly those with dementia or disabilities. Provide education and support to patients and their families regarding health management, including family planning options. Utilize electronic health record systems (FireNote) for documentation and patient management. Collaborate with other healthcare professionals in hospice medicine to ensure comprehensive care delivery. Maintain a safe environment for patients by adhering to infection control protocols and safety guidelines. Skills Proficiency in using electronic health record systems (FireNote). Experience in dementia care and working with individuals with disabilities. Knowledge of family planning practices, bereavement and patient education techniques. Excellent communication skills for effective interaction with patients, families, and healthcare teams. Ability to work in fast-paced environments while maintaining attention to detail.
