Qualifications :

Degree:

Associates or Baccalaureate Degree in Nursing.

Education:

Graduate from a college or university accredited by the Accreditation Commission for Nursing Education (ACEN) or the Commission on Collegiate Nursing Education (CCNE).

Experience:

Possess a minimum of one year of experience within the last two years working as a full-time RN-Case Manager.

Certification:

Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM) American Nurses Credentialing Center Nurse Case Manager (RN-NCM)

National Academy of Certified Care Managers:

Care Manager Certified (CMC)

Additional Certification:

Contract workers shall maintain current certification in Basic Life Support (BLS) from the American Heart Association, or the American Red Cross. Web-based (on-line) classes are not acceptable by the

Government Core Duties:

Duties shall consist of, but are not limited to, the following: Ensure Service Members (SMs) are medically cleared for Mobilization/Demobilization (MOB /DEMOB) or ensure SMs are sent to the proper sections or units for their care. The use of SOP's and published regulations are essential to succeeding in this environment. Provide these services in support of all Service Members who are seen at the facility and who are identified as having a medical/surgical or mental health issue when processing through for mobilization/deployment or demobilization/re-deployment for any contingency operations. Shall see an average of 10 to 30 SM's weekly and/or the weekly patient average of the other collective CPs, unless affected by circumstances beyond their control, which shall be reported to the Section OIC/NCOIC or designee. Unit trackers shall be updated daily. Shall assist in identifying service member needs and implementing the plan of care. Shall maintain documentation of all treatment provided in accordance with directives and prepare such records and reports as may be required by the policy. Documentation in MHS Genesis shall include, but is not limited to, all conversations whether in person or telephonic, guidance, recommendations you provide to SMs and their command teams. This includes any guidance, recommendations, and/or orders from the Provider(s). Shall prepare all documentation to meet or exceed established standards to include but not limited to timeliness, legibility, accuracy, content, and signature. Shall communicate pertinent service member data to Physician, Physician Assistant (PA), and /or Nurse Practitioner (NP) as appropriate.
