Job Overview We are seeking a dedicated PRN Registered Nurse (RN) Hospice Case Manager to join our team. This role offers flexible scheduling and the opportunity to provide vital hospice care services in patients' homes. The ideal candidate will possess extensive clinical experience, including critical care, ICU, or emergency medicine backgrounds, and demonstrate a strong commitment to compassionate patient-centered care. As a PRN RN Hospice Case Manager, you will coordinate comprehensive care plans, assess patient needs, and support families through difficult times with professionalism and empathy. Responsibilities Conduct thorough patient assessments to determine eligibility and develop individualized hospice care plans aligned with medical and emotional needs. Coordinate interdisciplinary team efforts to ensure seamless delivery of hospice services, including symptom management and comfort care. Provide education and support to patients and families regarding disease progression, medication management, and end-of-life planning. Monitor patient progress regularly, adjusting care plans as necessary to optimize comfort and quality of life. Document all clinical interactions accurately in medical records while adhering to HIPAA regulations and confidentiality standards. Collaborate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive patient support. Assist with discharge planning by coordinating post-acute or outpatient services when appropriate. Requirements Current RN licensure in the state of practice with a valid license; additional certifications such as Hospice or Palliative Care certification are preferred. Demonstrated experience in critical care settings such as ICU, emergency room, or trauma centers; experience in hospital medicine or post-acute care is highly valued. Strong knowledge of physiology, medical terminology, and diagnostic evaluation processes. Proven ability to perform comprehensive patient assessments and manage complex cases involving acute or chronic conditions. Familiarity with healthcare regulations including HIPAA compliance and documentation standards for medical records. Excellent communication skills for effective patient service delivery and interdisciplinary collaboration. Experience with discharge planning, utilization management, case management, or managed care environments is advantageous. Ability to work independently within a flexible schedule while maintaining high standards of patient care.
