Job:

Our census is increasing, and we are currently searching for a compassionate, empathetic, and experienced Full-Time RN Case Manager for our Garland Service Area, primarily in the Collin county area. Your primary focus will be to provide education, support, and clinical care to individuals and their families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life.
Responsibilities:
Perform and coordinate field visits with the nursing team and as needed by the patient's plan of care, and in an effort to support their family. Train and educate patients, family/caregivers, and staff on appropriate usage of medications, DME, and supplies as well as the disease progression. Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention. Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language that the patient and family understand. Demonstrate excellent interpersonal and communication skills. Qualifications:
Degree in Nursing (ADN or BSN). Valid RN Texas License. At least 1 year of Hospice experience. Valid Texas Driver's License and Proof of Auto Insurance. Reliable Transportation. Valid BLS. Successful completion of Background Screen, including employment verification. Schedule:
Day shift. Monday to Friday.
License/Certification:
RN License (Required). BLS Certification (Required).
