What You'll Do:
Provide skilled nursing care to home health patients in accordance with established care plans Perform comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized plans of care Monitor patient conditions and communicate changes to physicians and care team members Coordinate and oversee patient services to ensure continuity and quality of care Educate patients and families regarding disease management, medications, treatments, and home safety Supervise and direct licensed and non-licensed clinical staff as appropriate Complete timely and accurate clinical documentation and charting Conduct chart reviews for active and discharged patient records to ensure compliance and quality standards Participate in after-hours and on-call coverage as assigned Coordinate scheduling and staffing needs as required Maintain patient confidentiality and comply with all HIPAA regulations Stay current on nursing best practices, regulations, and clinical innovations #zr Current Registered Nurse (RN) license in the State of Texas Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or related healthcare setting Strong clinical assessment and critical-thinking skills Ability to work independently with minimal supervision Excellent communication and interpersonal skills Strong organizational and time-management abilities Commitment to maintaining confidentiality and professional integrity Previous home health experience preferred but not required Benefits Eligible employees may enjoy: Medical Insurance Dental Insurance Vision Insurance Life Insurance Flexible Spending Account (FSA) Paid Time Off (PTO) Retirement Plan Tuition Reimbursement Schedule Full-Time Part-Time PRN Day, Evening, and Night Shifts Available Build Your Future in Healthcare with Coryell Health Be part of something meaningful.
