Role:
As our MDS Coordinator, you'll coordinate and support the completion and timely submission of accurate interdisciplinary MDS assessments, CAAs, and Care Plans. You'll ensure everything aligns with CMS RAI Manual regulations, state and federal law, and company standards. This is a key clinical role where your analytical skills and nursing knowledge drive quality outcomes.
What You'll Do:
Coordinate and complete MDS assessments, CCAs, and Care Plans on time and accurately Monitor and report resident condition changes to the Director of Nursing Train direct care staff on CMS RAI Manual updates and regulatory shifts Review care plans and validation reports to ensure compliance and accuracy Notify the Business Office when assessments are ready (or not ready) for billing Analyze CMS reports and implement solutions to address late submissions Participate in PPS, Triple Check, and other interdisciplinary meetings
What You Bring:
Active nursing license in Texas Associate's or bachelor's degree in nursing from an accredited college or university At least 2 years of nursing experience (geriatric experience preferred) RAC-CT certification preferred Strong working knowledge of CMS regulations and MDS procedures Independent decision-making skills and a sharp attention to detail Solid communication and a team-first attitude Why Tide HG?
