In this vital role, you will provide expert nursing care across diverse clinical settings, ensuring the highest standards of patient safety, comfort, and well-being. Your expertise will support a wide range of medical services, from acute care to long-term and community-based care, empowering patients on their health journeys. Duties Proven experience as a Registered Nurse in diverse clinical environments such as hospital, home health, or outpatient clinics. Demonstrated proficiency with Kinnser EMR/EHR systems. Perform initial and ongoing nursing evaluations. Complete OASIS data sets accurately and timely, ensuring patients meet home health eligibility and medical necessity guidelines. Directly supervise and provide clinical direction to Licensed Practical Nurses (LPNs/LVNs) and Home Health Aides (HHAs). You will assign tasks and conduct required supervisory visits to ensure quality and continuity of services . Accurately reconcile medications during visits and communicate any discrepancies to the physician. Administer medications (including IVs), perform wound care and vacuum-assisted closure (VAC), manage central lines, tracheostomies, and ventilators, as well as ostomies and PEG tubes. Coordinate with multidisciplinary teams for discharge planning, case management, and long-term care arrangements in settings such as assisted living, nursing homes, or home & community care environments. Engage in patient education on health coaching topics including chronic disease management (diabetes, infectious disease care), medication administration, and lifestyle modifications. Maintain compliance with infection control standards and laboratory procedures for specimen collection/processing while ensuring accurate ICD coding (ICD-9/ICD-10) for billing purposes. Experience Minimum of 1-2 years of recent experience in home health, acute care, or public health nursing .
Technical Knowledge:
Familiarity with Medicare home health regulations, OASIS documentation, ICD-10 coding, and Electronic Medical Records (EMR) with Kinnser/Wellsky is strongly preferred .
Soft Skills:
Ability to work independently, excellent written/verbal communication, strong organizational skills, and critical thinking under pressure. Milleage reimbursement is offered.
Pay:
$43.00 - $90.00 per hour
