  • Trauma Services Houston, TX Full-Time | Onsite Requirements Current and valid Texas RN license or Licensed Master Social Worker (LMSW) LCSW preferred Case Manager Certification ( CCM or equivalent) required Minimum 5 years of experience in utilization management, case management, discharge planning, or related care management programs Minimum 3 years of hospital-based nursing or social work experience Minimum 3 years of leadership or management experience Strong knowledge of care coordination, utilization review, discharge planning, and regulatory requirements Trauma services experience LCSW certification Experience managing hospital-based case management teams Key Responsibilities Lead and oversee daily case management operations for trauma services Manage and mentor case managers and social work staff Ensure effective discharge planning and patient throughput Monitor utilization management and resource optimization initiatives Collaborate with physicians, nursing leadership, and interdisciplinary teams Drive quality improvement and patient outcome initiatives Ensure compliance with hospital, state, and federal regulations Analyze departmental metrics and implement performance improvement strategies
