  • 1:00 PM We are seeking a dedicated and experienced LVN to provide one-on-one care for an adult patient in a home health setting.
The ideal candidate will have hands-on experience with tracheostomy (trach), ventilator (vent), and G-tube care , and a strong commitment to delivering safe, high-quality care. Responsibilities Provide skilled nursing care in the patient's home Manage and monitor tracheostomy and ventilator support Perform G-tube feedings and site care Administer medications and treatments as prescribed Monitor and document patient status, vital signs, and changes in condition Suctioning and airway management as needed Maintain accurate and timely clinical documentation Collaborate with family members and healthcare team Ensure infection control and patient safety at all times Qualifications Current Texas LVN license (required) Experience with trach, ventilator, and G-tube care (required/preferred) Current CPR certification Strong clinical assessment and critical thinking skills Dependable, compassionate, and detail-oriented Home health experience preferred Benefits (optional—customize as needed) Competitive pay Flexible scheduling Supportive clinical team Ongoing training opportunities
