1 Description American Traveler is hiring an experienced Hospice RN for a home health and hospice agency in Kyle, TX, requiringHCHB/OASIS
experience and the ability to perform same-day admissions. Details Home health and hospice setting serving patients across a wide geographic territory in central Texas Caseload of approximately 4-6 patients per day, with a target of 30 points per week (equivalent to 40 hrs/week) Patient population includes geriatric and in-home hospice patients requiring skilled nursing and interdisciplinary care coordination All charting must be completed on-site at the patient's home using HCHB and OASIS Admissions are required as part of this role; must be prepared to see patients same-day Schedule is Monday-Friday, 8:00 AM-5:00 PM (5x8-hour shifts) Weekend availability required as needed On-call rotation is required, including approximately one overnight call shift per week Must be willing to travel up to 50 miles from the home base office Uses Homecare Homebase (HCHB) EMR system Requirements Active TX RN license or Compact RN license currently in hand — pending licensure is not accepted Current BLS certification required Minimum 1 year of hospice nursing experience required; minimum 2 years of overall clinical experience required Proficiency in HCHB and OASIS required; 1 year of experience with both is preferred Must be comfortable performing same-day patient admissions 2 professional supervisory references from within the past 2 years required for consideration Full 7-year work history with all gaps explained required for consideration Additional Information Responsible for comprehensive patient assessments, plan of care development, and ongoing care coordination with the interdisciplinary team and certifying provider Provides skilled nursing care to hospice patients in a variety of home environments across a broad service area Non-smokers strongly preferred, as many patients have COPD and are sensitive to the smell of cigarette smoke Scrubs in any color are acceptable This is a privately owned, female-founded hospice and home health organization headquartered in central Texas with multiple regional offices Local candidates within a 50-mile radius are accepted First-time travelers are welcome to apply All scheduling or time off requests must be disclosed at the time of initial considerationSimilar jobs in Kyle, TX
