Job Description

Job Summary We are seeking a dedicated and detail-oriented MDS Coordinator - LVN (Licensed Vocational Nurse) to join our healthcare team. In this vital role, you will coordinate and facilitate the completion of the Minimum Data Set (MDS) assessments for our long-term care residents, ensuring compliance with federal and state regulations. Your expertise will support accurate clinical documentation, optimize patient care plans, and enhance overall quality of care delivery. Duties Conduct comprehensive MDS assessments in accordance with CMS (Centers for Medicare & Medicaid Services) guidelines and ensure timely completion for each resident. Collaborate with interdisciplinary teams including nursing, therapy, social services, and medical staff to gather necessary clinical information. Review medical records, physician notes, and other documentation to ensure accuracy and completeness of data used for MDS coding. Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document assessments and update patient records efficiently. Ensure compliance with HIPAA regulations by maintaining confidentiality of all resident information during documentation review and data entry. Assist in the development of individualized care plans based on MDS data to improve patient outcomes and meet regulatory standards. Participate in quality assurance activities related to clinical documentation improvement, utilization review, and coding accuracy. Requirements Valid LVN license with current state licensure required; additional certifications in medical coding or case management are a plus. Proven experience with MDS coordination in a long-term care setting or skilled nursing facility. Strong knowledge of CMS regulations, NCQA standards, and Medicare requirements related to MDS and utilization management. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; experience with clinical documentation improvement initiatives preferred. Knowledge of medical terminology, anatomy, physiology, ICD-10/ICD-9/ICD coding, CPT coding, DRG assignment, and health information management practices. Critical care experience including ICU or Level I/II trauma center experience is highly desirable; inpatient or hospital experience is beneficial. Excellent organizational skills with the ability to prioritize multiple assessments while maintaining attention to detail. Strong communication skills for effective collaboration across multidisciplinary teams and clear documentation review.
