Director of Nursing (DON) / Clinical Supervisor Position Summary The Licensed Vocational Nurse (LVN) - Quality Assurance (QA) is responsible for reviewing and auditing home health patient charts to ensure compliance with CMS, Medicare, Medicaid, state, and agency regulations. The LVN QA Nurse will review clinical documentation including OASIS assessments, Start of Care (SOC), Recertifications, Resumption of Care (ROC), Transfer, Discharge, and routine skilled nursing visit documentation for accuracy, completeness, and regulatory compliance. The QA Nurse works closely with clinicians and office staff to improve documentation quality, reduce billing issues, and support survey readiness. Essential Duties and Responsibilities Perform quality assurance review of patient charts for completeness, accuracy, and compliance with CMS and agency guidelines. Review and audit OASIS documentation including SOC, ROC, Recertification, Transfer, and Discharge assessments. Ensure documentation supports medical necessity, homebound status, and skilled need requirements. Review physician orders, Plan of Care (CMS-485), medication profiles, frequencies, goals, and interventions for consistency and compliance. Verify documentation meets Medicare, Medicaid, Palmetto GBA, and state regulatory standards. Identify documentation deficiencies, inconsistencies, missing signatures, incomplete orders, or compliance concerns and communicate findings to clinicians promptly. Assist field clinicians with corrections, chart updates, and documentation education as needed. Review skilled nursing, therapy, HHA, and interdisciplinary notes to ensure appropriate continuity of care. Monitor timeliness of documentation submission and physician order tracking. Participate in agency performance improvement and quality assurance initiatives. Assist with preparation for state surveys, accreditation reviews, and internal audits. Maintain confidentiality and HIPAA compliance at all times. Collaborate with intake, scheduling, billing, and clinical staff to ensure accurate patient records and claim readiness. Qualifications Current and active Texas LVN license in good standing. Minimum of 1-2 years of home health experience preferred. Experience with OASIS documentation and QA review required. Knowledge of CMS, Medicare Conditions of Participation, and home health regulations. Experience reviewing SOC, ROC, Recertification, and discharge documentation preferred. Proficient in Kinnser/WellSky or other home health EMR systems. Strong attention to detail and organizational skills. Ability to work independently and meet deadlines. Strong communication and problem-solving skills. Preferred Skills Knowledge of ICD-10 coding and diagnosis sequencing. Familiarity with billing readiness and claim review processes. Ability to educate clinicians on compliant documentation practices. Experience with chart audits and quality improvement initiatives.
Pay:
$18.00 - $20.00 per hour
