Position Summary The Hospice RN Case Manager is an integral member of the interdisciplinary team and is responsible for coordinating and managing the care of assigned hospice patients and families. This role provides skilled nursing assessments, develops individualized plans of care, and collaborates closely with physicians and other team members to ensure exceptional end-of-life care. The Hospice RN Case Manager works closely with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other healthcare professionals to promote comfort, dignity, and quality of life for patients and their loved ones. Responsibilities Patient Care & Case Management Assume primary responsibility for an assigned patient and family caseload. Complete comprehensive nursing assessments and develop individualized plans of care. Coordinate care with physicians and members of the interdisciplinary team. Provide regular visits to reassess patient needs and update care plans as appropriate. Manage symptoms and provide skilled nursing interventions to maximize comfort and quality of life. Educate patients and families regarding disease progression, medications, symptom management, and hospice services. Provide emotional support and guidance to patients and families throughout the hospice journey. Perform clinical pronouncements and provide compassionate support to families following a patient's death. Participate in interdisciplinary team meetings and collaborate to ensure continuity of care. Maintain timely and accurate documentation in accordance with agency standards and regulatory requirements. Compliance & Documentation Adhere to Medicare Hospice Conditions of Participation and agency policies and procedures. Complete all clinical documentation accurately and within required timeframes. Ensure patient records are maintained in accordance with regulatory and accreditation standards. Qualifications Required Current Texas Registered Nurse (RN) license in good standing. Minimum of two years of nursing experience. Hospice or palliative care experience preferred. Strong assessment and clinical skills. Excellent communication and interpersonal skills. Ability to work independently while collaborating effectively with the interdisciplinary team. Compassionate, patient-centered approach to care. Valid driver's license and reliable transportation.
