  • Part-time, Full-time, PRN, Per diem Job Overview Join our compassionate team as a Hospice Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in the lives of patients facing life-limiting illnesses.
In this vital role, you will provide skilled nursing care, coordinate patient assessments, and support families through challenging times. Your energetic approach and dedication will help deliver comfort, dignity, and personalized care in a hospice setting. This position offers an opportunity to work closely with interdisciplinary teams, utilizing your critical care experience and specialized skills to enhance patient well-being and ensure seamless care transitions. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and symptom management to evaluate ongoing needs. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Manage airway care, tube feeding, catheterization, and ventilator support for patients requiring advanced interventions. Document all patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner; ensure compliance with HIPAA regulations. Collaborate with physicians, social workers, case managers, and family members to develop and update individualized care plans aligned with hospice philosophy. Provide education on pain management techniques, dementia and Alzheimer's care strategies, and end-of-life comfort measures. Assist with medical documentation related to
ICD-9/ICD-10
coding for billing purposes; participate in discharge planning and post-acute care coordination. Requirements Valid LVN license in the state of employment with current certification. Proven experience in hospice care or related fields such as critical care, ICU, emergency medicine, or medical-surgical units. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to end-of-life care. Hands-on experience with EMR/EHR systems like Epic or Athenahealth; proficient in medical documentation and medical records management. Skilled in patient assessment techniques including vital signs measurement and diagnostic evaluation. Ability to perform IV insertions, phlebotomy, injection administration, catheter care, and sterile technique procedures confidently. Familiarity with infection control practices and HIPAA compliance standards. Excellent communication skills to work effectively with patients of diverse backgrounds—including those with developmental disabilities—and their families. Experience working in various settings such as long-term care facilities, assisted living communities, hospitals (including Level I/II Trauma Centers), nursing homes, or home & community-based care environments. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps is a plus. Physical ability to perform patient transfers using Hoyer lifts or other assistive devices; capable of working flexible hours including on-call shifts.
