  • Full-time FT Case Manager / MDS Nurse (RN/LVN) We are seeking a dedicated and organized RN or LVN to join our team as a Full-Time Case Manager / MDS Nurse.
This hybrid role is ideal for a nurse who enjoys both clinical coordination and resident assessment responsibilities. MDS experience is preferred but not required. Responsibilities Coordinate resident discharge planning to ensure smooth transitions of care. Complete and submit insurance updates and documentation in a timely manner. Perform MDS assessments and assist with care plan development. Collaborate with interdisciplinary team members to support resident care needs. Maintain accurate and compliant clinical documentation. Assist with additional case management and MDS-related duties as needed. Remain flexible and support facility operations based on census and resident needs. Qualifications Current Texas RN or LVN license in good standing. MDS experience preferred. Knowledge of Medicare, managed care, and insurance processes preferred. Strong organizational and communication skills. Ability to work independently and as part of a team. Long-term care or skilled nursing experience preferred.
