  • We are seeking a dedicated and compassionate RN Case Manager to join our dynamic healthcare team. The ideal candidate will excel in coordinating patient care, ensuring optimal outcomes, and facilitating the efficient use of resources.
  • Key Responsibilities:
  • Collaborate with patients, families, and interdisciplinary teams to develop and implement individualized care plans.
Conduct comprehensive assessments to determine the needs, preferences, and goals of patients. Monitor patient progression, adjusting care plans as necessary to ensure effective outcomes. Serve as a liaison between patients, healthcare providers, and insurance companies to coordinate services and resources. Educate patients and families about treatment options, care plans, and health management strategies. Maintain accurate and up-to-date patient records in compliance with healthcare regulations and standards. Advocate for patients' needs, ensuring they receive appropriate and quality healthcare services.
  • Qualifications:
  • Current and valid Registered Nurse (RN) license. Proven experience in case management or related healthcare field. Strong organizational skills and attention to detail. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of healthcare regulations and compliance.
  • Preferred Qualifications:
  • Certification in case management (CCM, ACM, or equivalent).
